Please fill out the registration form completely and print clearly. You will need to turn in a registration form for EACH child. Please mark the session your child will attend. If you want your child placed on a team with a friend of the same age, please indicate that preference in the section labeled "Placement Requests." Registrations are handled on a "first-come, first-served" basis. We encourage you to register early as spots do fill quickly. Each registration includes a camp Tee shirt and all supplies needed.

If you want to order a DVD, the cost is $30.00(this includes shipping and sales tax). Please include this amount in your check.

If you would like to sponsor a child, please indicate on your check that it includes a donation.

Make checks payable to Joanie's Art Studio and mail to 1100 Vassar St., Orlando, FL 32804.
